GLP-1 Microdosing: What It Is and What We Actually Do

GLP-1 microdosing means fractional dosing individualized to response and tolerance. Evidence remains limited, and at Rivas every off-label dosing decision is made by a licensed provider—not by a DIY formula.

Prefer clinician guidance?

Dosing decisions belong with your provider.

A licensed clinician determines whether any dose or delivery method is appropriate for you.

Educational, off-label reference only: Manufacturer guidance does not recommend click counting, and clinical guidelines do not support it as a dosing protocol. This tool is not medical advice. Do not start, stop, or change medication based on this result; follow your prescription, manufacturer instructions, and licensed clinician. Compounded medications are not FDA-approved. Rivas does not recommend dosing based solely on pen clicks.

Safety & candidacy

Am I a good candidate for provider-supervised GLP-1 titration?

Your Rivas provider evaluates you at your first visit and makes the determination. The information below offers general context about who this treatment may fit.

provider-supervised GLP-1 titration may be a good fit if

  • You want a licensed provider to evaluate whether individualized GLP-1 titration fits your clinical needs.
  • You have experienced gastrointestinal intolerance or an unusually pronounced response that may warrant closer dose review.
  • You are transitioning between medications or have a clinical circumstance that may require a provider-directed temporary adjustment.
  • You value regular in-person monitoring and provider-administered treatment.

provider-supervised GLP-1 titration may not be appropriate if

  • Microdosing is off-label and lacks randomized controlled trial evidence as a standalone protocol.
  • It should not be used for vague wellness claims, self-directed experimentation, or medication obtained without clinical oversight.
  • Compounded GLP-1 medications are not FDA-approved finished products or equivalent to Wegovy or Zepbound.
  • Eligibility and every dosing decision depend on an individualized provider evaluation, including medication-specific contraindications.

Side effects and their frequency vary by medication and patient. Your provider will review risks, contraindications, and medication interactions during your evaluation.

The clinical definition

What “microdosing” a GLP-1 actually means

Published medical commentary describes GLP-1 microdosing as administering fractional doses smaller than standard protocols and titrating them to an individual patient’s tolerance and response.

It does not mean taking a minimal dose for vague wellness benefits or calculating a dose from social media. Microdosing is off-label, is not established in clinical guidelines, and has not been tested as a standalone protocol in randomized controlled trials.

Rivas uses compounded medication from multi-dose vials in appropriate cases. This can permit precise provider-directed increments, but compounded medications are not FDA-approved finished products and are not the same as brand-name Wegovy or Zepbound.

Why regular clinical contact matters

Your provider reviews actual response and tolerability before deciding whether any adjustment is appropriate.

Delivery formats

Pen-based and vial-based dosing are not the same

Pen-Based Dosing

Brand-name pens are designed around manufacturer-specified products and doses rather than arbitrary fractional increments.

Vial-Based Rivas Care

In most cases, the provider draws a precise dose from a multi-dose vial and administers it in the office.

Seen regularly

Precision comes from the provider relationship

A vial alone does not create individualized care. Regular contact lets your provider understand your history, monitor response, catch problems early, and decide whether to hold, raise, lower, or change treatment. Individual results vary.

01In-office evaluationYour provider reviews health history, medications, metabolic status, and prior treatment response before discussing GLP-1 care.
Clinical review includes
Medical and medication history
Prior GLP-1 response
Contraindications and treatment goals
02Plan the starting approachThe provider selects a clinically appropriate medication and starting plan; microdosing is not applied as a default protocol.
The plan addresses
Compounded versus brand-name options
Off-label status and limited evidence
Individual tolerance and response
03Provider-administered startIn most cases, your provider draws the prescribed amount from a vial and administers it during the office visit.
Starting safely means
No DIY dose calculations
Medication-specific instructions
Questions reviewed with a provider
04Response-based follow-upMost patients are seen frequently early in treatment, with later timing determined by the provider and clinical response.
Each review considers
Progress and tolerability
Whether to hold or adjust

Step 2

Discuss the evidence

Review the limitations and risks of off-label microdosing and whether it is appropriate for you.

Step 3

Follow a monitored plan

If recommended, follow provider-directed dosing with regular checks on response and side effects.
